Please complete the on-line preregistration materials, and then call one of the registrars below for an appointment.  Please return the completed attachments at the time of your appointment.  If you have any questions on the attached forms or need assistance, we would be happy to help you complete the forms at the time of your appointment.  Your child will not be officially enrolled until after your appointment and all information has been verified by the registrar.

Middle & High School - Grades 5 - 12 - contact Mrs. Julie Chartrand @ 315-543-2920 X4 or jchartrand@hcsk12.org

Please provide the following information at the time of your appointment with the registrar:

1. Parent/guardian photo I.D.

2.  Student birth certificate or state or government issued I.D. or any official record providing date of birth.

3.  Proof of immunizations.

4.  Custody paperwork must be provided.

5.  Proof of address showing name and address.

6.  If military, have brigade information or military I.D. available with you.
