REQUIRED TO COMPLETE REGISTRATION
1. Child's birth certificate/baptismal cert. (IF not available the following can be accepted: driver's license; state or government issued ID; school photo ID w/ DOB; consulate ID card; hospital or health records; military dependent ID; documents issued by federal, state, or local agencies; court orders or other court-issued documents; Native American tribal documents; records from non- profit international aid agencies and voluntary agencies) 2. Immunization record (with drs. signature OR stamp) 3. Physical (done within the last year) 4. TWO Proofs of residency lease agreement Mortgage Statement Statement from landlord 2ND PROOF OF RESIDENCY WILL NEED TO BE ONE OF THE FOLLOWING: Pay stub; Income tax form; utility bill; driver's license w/ insurance card, learner's permit, or non-driver ID State or government issued ID A court issued document
5. IEP (if applicable, would be helpful) PLEASE MAKE SURE YOU SIGN THE SPECIAL ED REQUEST OF RECORDS FORM IN THE "ATTACHMENTS" 6. Under the "ATTACHMENTS" below - you will download the grade appropriate registration forms (Elementary 1-5; JSHS 6-12). 4 year olds will be PreK registration paperwork. 3 year olds need to download the EPK PreK Program Form.
